How Studies Define Treatment Success
Complete Response
The strictest definition: after treatment, the apnea-hypopnea index falls below a threshold commonly considered normal or near-normal on follow-up sleep testing. Studies using this definition report lower success rates because they demand that nearly all measurable breathing events resolve.
Partial Response
A more permissive definition: the AHI falls by a defined percentage from the baseline diagnostic study — a 50% reduction is a commonly used benchmark. A patient whose AHI drops substantially but not completely to the normal range counts as a success under this definition but not under complete-response definitions.
Symptom-Inclusive Definitions
Some studies add subjective criteria — reduced daytime sleepiness or improved quality-of-life measures — either alongside or instead of AHI thresholds. These definitions capture whether patients feel better, but they can miss residual breathing events that symptoms alone do not reveal.
Adherence-Inclusive Definitions
Real-world effectiveness depends on the appliance being worn. Some research reports outcomes only for patients who used the appliance consistently, while effectiveness data for inconsistent users is reported separately. This distinction is explored in our adherence guide.
What Can Influence Reported Rates
Beyond the definition of success itself, reported rates are shaped by the study population (severity mix, age, body habitus), the appliance type and how it was titrated, whether titration was protocol-driven or ad hoc, how long outcomes were followed, and whether follow-up verification used home testing or in-lab polysomnography. Each of these can move the reported rate without changing what an individual patient experiences.
